If I provide my email address on the rebate application or on the website for the online application, will you use it for anything other than rebate verification?

When will I receive payment?
Once rebate submission is received, it could take up to 8 weeks to receive the rebate MasterCard® prepaid card. Please allow 4-6 weeks after mailing submission to make inquiries.
What are my payment choices?
Your rebate payment will be in the form of a prepaid card. Rebate cards are processed by MasterCard® only. Normally, rebate payments are received 6-8 weeks after approval.
Whose name will appear on the rebate card?
The name on the rebate card will be the same as the name on the rebate application. This name must match the billing information on the invoice as stated in the terms and conditions of your rebate offer.
How do I change the name on the rebate card?
The rebate card will only be made out to the name on the rebate application. This name must match the billing information on the invoice as stated in the terms and conditions of your rebate offer. There is no way to have it made out to another name.
Where will my rebate be sent?

How long is the prepaid card valid
Your MasterCard® prepaid card is valid for 6 months upon the issue.
How can I check the status of my rebate?

Registering for a Rebate
How do I claim a rebate?
Enter the required information and print the online rebate form. Sign the printed form and mail it to the specified address along with all required documentation. You will be able to track your rebate status on our website at https://tracktargets.rebateresearch.com/.
I did not get a rebate form with my product. Can I still apply for my rebate?
Yes. The product rebate application can be found on this website.
Why do I have to mail in the form and information?
It ensures that all the information required to validate your rebate has been accurately recorded. The retailer requires that you mail in the rebate form and supporting documentation to verify that the correct product was purchased during the rebate promotional period.
Do I need to use multiple envelopes when submitting more than one rebate?
You must use a separate mailing envelope and form. The retailer must keep individual records of each submitted rebate form. Even if more than one of the same rebate offer code is being submitted, each must be mailed in their own envelope. Be sure to read the requirements for each application and sign all rebate forms.
How do I obtain an offer code?
A promotional/offer code is not required for this rebate.
Can I use a packing slip in place of a receipt?
In some instances packing slips can be substituted for an invoice or sales receipt; in these cases, the rebate terms will indicate a packing slip may be used. If accepted, the packing slip must include your bill to and ship to address, the item purchased which qualifies for the rebate, the place of purchase and date of purchase. All information must meet the requirements as stated in the terms and conditions of the rebate offer.
What is a proof of purchase?
The proof of purchase (also commonly referred to as a UPC) is used to show that the correct item for the rebate was purchased. It is a barcode located on the product.
What is a UPC and where is it located?
The UPC barcode is located on the package or product. Each item has a unique UPC, so it is necessary to attach the correct one when filling out the rebate documentation.
How can I obtain an invoice/sales receipt?
You should contact your place of purchase to request an invoice/sales receipt. Most retailers can reprint a sales receipt for you, especially if you paid with a credit card and know the exact date of your purchase. In some instances, a packing slip may be substituted for an invoice/sales receipt. If accepted, the packing slip must include your bill to and ship to address, the item purchased which qualifies for the rebate, the place of purchase and date of purchase.
Does my invoice/sales receipt have to match my rebate mailing address?

Do I need to have a computer to submit a rebate?
You will need access to a computer in order to obtain your rebate form. You will need to mail the rebate form you have printed and signed with proper documentation.
TERMS AND CONDITIONS
Purchases must be within the promotional time period 08/01/2020 through 09/30/2020 and mailed with a postmarked by 10/15/20. Please keep a copy of all materials submitted for your records. Duplicate requests will not be honored and will not be returned. Rebates will not be honored without the submission of the completed rebate form, the original receipt, and the original UPC from each product purchased. Submissions must be within the promotional time period; incomplete or illegible submissions will be denied. The purchase date on your sales receipt must be a date within the eligible period indicating on the individual rebate form that you must print, complete and mail to the specified address. The address form must match the billing address on the receipt. Any requests postmarked or received after the eligibility period will be denied. Rebates will be issued in the form of a MasterCard Prepaid Card. The card will be valid for up to (6) six months. Once the card is expired, the card is void and will not be replaced. This offer is not available to any clubs, groups, organizations or dealers. CONSUMER REBATE ONLY. US entries only. See website for entire rules and regulations.
